1) Section of the colon showing foamy macrophages in the lamina propria.

2) Ziehl - Neelson stain of section 1) showing numerous bacilli.

3) PAS positive stain of section 1) is also clearly demonstrating the bacilli.

4) Section of the liver showing discrete non-necrotic clusters of macrophages in the sinusoid.

5) Ziehl - Neelson stain of the section 4) is showing intracellular acid fast bacilli.

 

Diagnosis: This is a case of disseminated infection with Mycobacterium Avium Intracellulare (MAI). Multiple organs are affected in this HIV positive case.
